Are You a Carer?

A carer is a person of any age (including children) who provides unpaid support to a partner, parent, relative, friend or neighbour who couldn’t get by without their help. This may be due to old age, frailty, disability, a serious health condition, mental illness or substance abuse.

Many carers do not see themselves as carers because they see themselves as a parent, a spouse or partner, a son or daughter, a relative, friend or neighbour.

BUT 1 in 10 of the population is a carer.   And as a result many carers often miss out on services, support, advice and benefits which may be available and may help.

Once you register as a Carer with the Practice, one of our PCN Care Co-ordinators will contact you to complete a Carers Assessment and to ensure you, and the person you care for, are getting everything you need. The Care Co-ordinators are on a mission to ensure everyone is getting the best from Bridge Medical Centre and will be calling patients to get them booked in for outstanding vaccines and screenings and to ensure you have the support to manage your healthcare needs.

We can also refer you to a Carer support. They can provide you with local and national information on services available including benefits and financial information as well as local events and activities for carers. They can arrange emotional support which includes counselling sessions with specially trained counsellors, a telephone call back service as well as other support for emotional wellbeing and relaxation.

 

Supporting Carer’s in General Practice

Our Declaration

We are proud to announce that Bridge Medical Centre has worked through the NHS England initiative of “Supporting Carer’s in General Practice”.  A voluntary project team was formed from different areas of the Practice who came together to work through six quality markers with a view to improving carer’s health-related quality of life and reducing carer breakdown.  We are now proud to publish the current Declaration setting out the key actions and changes we have made. It has been a very rewarding project and has touched many of the staff who have worked towards our published Declaration.
