Tracy is our New Social Prescriber at Bridge Medical Centre

Here Tracy explains about her role and what help she can offer to patients'

"I started work as a Social Prescriber for prescription Plus a Crawley Community Action Project in 2016. I am now working for the Crawley Care Collaborative PCN which includes the following Practices in the Crawley area, Bridge Medical Centre, Langley Corner Surgery, Leacroft Medical Practice and Southgate Medical Group. Many things can affect your health and wellbeing. Clinicians tell me that a lot of people visit them feeling isolated or lonely or stressed by work, money or housing related issues. Sometimes it’s the stress of managing different long-term health conditions. That’s where social prescribing comes in, it starts with a conversation. It might be the conversation you’ve just had with your clinician or with another person in the practice team. They will refer you, to me your practice social prescribing link worker, I can support you in finding appropriate low cost or free help or advice."
